If your physician believes you would benefit from marijuana use, they will submit a certificate to the Pennsylvania DHSS as soon as possible. Once your information is in the system, you can continue the application process on the DHSS website.

Missouri Qualifying Conditions

In order to be eligible for a Missouri medical marijuana card, you must have at least one of the state's qualifying conditions. As long as you have one of them and your doctor confirms that medical marijuana could improve your treatment regimen, you can qualify for a card.

Here are the qualifying conditions for patients in Missouri:

  • Cancer
  • Terminal illness
  • Epilepsy
  • Glaucoma
  • Intractable migraines
  • Chronic medical condition(s) with severe and persistent pain or muscle spasms, including but not limited to multiple sclerosis, Parkinson’s Disease, Tourette’s Syndrome, or seizures
  • Debilitating psychiatric disorder(s), including post-traumatic stress disorder (PTSD)
  • Human Immunodeficiency Virus (HIV) or Acquired Immune Deficiency Syndrome (AIDS)
  • Alzheimer's Disease
  • Cachexia
  • Chronic medical condition(s) usually treated with prescription medication(s) that involve the risk of physical and/or psychological dependence
  • Any other source(s) of chronic pain or any other debilitating medical condition(s) a physician would deem suitable for medical marijuana treatment, including Amyotrophic Lateral Sclerosis (ALS), inflammatory bowel disease, Crohn’s Disease, autism, neuropathies, sickle cell anemia, or wasting syndrome

Age & Residency

Every Missouri medical marijuana patient must be 18 years old or older, a Missouri resident for at least 45 days, and lack any disqualifying condition(s).


Medical Documentation

In order to obtain a medical marijuana card in Missouri, you will need to provide a letter from your doctor documenting your qualifying medical condition. The letter must be signed by the doctor and read as follows:

"I, (physician's full name) , am a licensed physician practicing medicine in the State of Missouri. I have a patient by the name of (your full name) who has been my patient for at least three months prior to signing this document. My patient has been diagnosed with ____________. In my medical opinion, the potential benefits of the medical use of cannabis would likely outweigh the health risks."

If your physician is unwilling to provide a letter verifying your condition, you may have another doctor fill out a written statement instead. If you are in hospice care or being treated for a terminal illness, an advanced practice registered nurse or a physician's assistant can provide the written statement.


Get Evaluated

If you're interested in getting a medical marijuana card, the first step is to get evaluated. To get evaluated, you need to go see a doctor who can recommend medical marijuana.

That doctor must write and sign a document affirming that you're suffering from at least one of Missouri's official qualifying conditions. You must present that document with your medical marijuana card application to be considered for a card.
