Forms

The DHSS will then receive this electronic form directly. The patient only needs one Physician Certification Form if they want the standard four doses per month. However, if the doctor advises a higher dosage, the patient will require the completion of two separate Physician Certification Forms.

A Parent/Legal Guardian Consent Form is also required if the patient receiving medical marijuana is under the age of 18. Patients must also complete a Patient Authorization Form if they intend to rely on a licensed caregiver to pick up their marijuana on their behalf.

After receiving the doctor's certification, you have 30 days to complete this registration and application process.

Register

The patient must register online through Missouri's DHSS portal after the doctor submits all of the necessary paperwork. Before continuing, they must choose the appropriate registration (patient/caregiver). They will already have a connection between their new account and their electronic Physician Certification Form.

Application

The patient or their caregiver must complete an application through the online portal after registering. After that, it will be processed by the DHSS in 30 days. Please be aware that Missouri does not accept applications on paper or in person.
