You do not need to have a prior doctor's evaluation to meet with our licensed physicians.

During your appointment, your doctor will evaluate your condition and determine if you meet the state’s requirements for medical marijuana use. If your physician believes you would benefit from marijuana use, they will submit a certificate to the Pennsylvania DHSS as soon as possible. Once your information is in the system, you can continue the application process on the DHSS website.

Forms

The forms for getting a medical marijuana card in Missouri can be found on the website of the Department of Health and Senior Services. The application form can be filled out online, or it can be printed out and filled out by hand. The forms require basic information like name, address, and date of birth.

The application also requires information about the patient's medical history, including any conditions that the patient is currently being treated for. The patient's current doctor must also provide a certification that confirms that the patient would benefit from using medical marijuana.


Register

The first step in the registration process is to register. The registration form will contain your name and address and will require you to specify whether you're a sick, disabled, terminally ill patient, caregiver or physician.

After this form has been submitted, it will be given a verification code to ensure the legitimacy of your application. Once you have been verified you will have to pay a fee for the card.

You'll need to submit a social security number or proof of identity in order to receive confirmation that your application was approved by the Missouri Department of Health and Senior Services' Bureau of Marijuana Control.


Application

In order to apply for a Missouri Medical Marijuana Card, you will need to provide the following information:

  • Your full name
  • Your date of birth
  • Your current zip code
  • proof of residency (this can be a driver's license, state ID card, or utility bill)
  • a written certification from a physician that you suffer from one of the qualifying medical conditions

Once you have gathered all of this information, you can submit your application online or by mail.

In the state of Missouri, medical marijuana is legal for those with a qualifying condition. In order to qualify for medical marijuana treatment, you must have been diagnosed by your doctor as having one of the state's official qualified conditions. The law also allows people who suffer from chronic pain and muscle spasms due to multiple sclerosis to apply for a medical marijuana card.

The process of obtaining this card includes filling out an application form and sending it in along with payment for the required fee. Read on to learn more about the application and how to find out whether you're eligible.

Missouri Qualifying Conditions

In order to be eligible to receive a Missouri medical marijuana card, you must be dealing with one of the qualifying conditions determined by the state government. Those conditions include:

  • Cancer
  • Terminal illness
  • Epilepsy
  • Glaucoma
  • Intractable migraines
  • Chronic medical condition(s) with severe and persistent pain or muscle spasms, including but not limited to multiple sclerosis, Parkinson’s Disease, Tourette’s Syndrome, or seizures
  • Debilitating psychiatric disorder(s), including post-traumatic stress disorder (PTSD)
  • Human Immunodeficiency Virus (HIV) or Acquired Immune Deficiency Syndrome (AIDS)
  • Alzheimer's Disease
  • Cachexia
  • Chronic medical condition(s) usually treated with prescription medication(s) that involve the risk of physical and/or psychological dependence
  • Any other source(s) of chronic pain or any other debilitating medical condition(s) a physician would deem suitable for medical marijuana treatment, including Amyotrophic Lateral Sclerosis (ALS), inflammatory bowel disease, Crohn’s Disease, autism, neuropathies, sickle cell anemia, or wasting syndrome

Age & Residency

In order to be eligible for a Missouri medical marijuana card, you must be a resident of the state and at least 18 years old.


Medical Documentation

If you want to obtain a Missouri medical marijuana card, you will need to obtain a document from a state-certified physician affirming that you're suffering from one of the state's official qualifying conditions.


Get Evaluated

Speaking to a medical marijuana doctor in Missouri is a different type of experience. There are many things that can alter the process and it's important to know what you're getting into before going through the evaluation.

The physician will have to be willing to put in a lot of time and effort for evaluations so there might be limited office hours or days for appointments. That’s why it’s a great idea to set up a telehealth appointment.

You should also be aware that you'll have to undergo multiple evaluations over the course of six months to determine if you qualify for a prescription from a doctor. This has to be done in order to receive a medical marijuana card in Missouri.

You will have to contact the doctor either via email or phone depending on their availability. You'll also need to make sure that your physician is available for an appointment at least once every month for six months. This means you should try to get in touch with the doctor at least a month before your current evaluation period ends.
