If your physician believes you would benefit from marijuana use, they will submit a certificate to the Pennsylvania DHSS as soon as possible. Once your information is in the system, you can continue the application process on the DHSS website.

How It Works
There are several forms that are required in order to get a medical marijuana card in Missouri. The first is the application form, which can be found on the website of the Missouri Department of Health and Senior Services. The second is the physician certification form, which must be completed by a licensed physician. The third is the release of information form, which must be signed by the applicant.
