If your physician believes you would benefit from marijuana use, they will submit a certificate to the Pennsylvania DHSS as soon as possible. Once your information is in the system, you can continue the application process on the DHSS website.

Forms

In order to get a medical marijuana card in Missouri, you will have to fill out a few forms. The first form is the application form, which you can find on the website of the Department of Health and Senior Services. The second form is the physician certification form, which your doctor will have to fill out. You can find this form on the website of the Department of Health and Senior Services as well. You may be able to find the physician certification form on your doctor's website.

The application form asks for your name, address, phone number, date of birth, and social security number. You can also choose what kind of ID you will like to use with your medical marijuana card. Your choices are a driver's license or non-driver ID or a US passport or military ID. You also have to choose the county you live in and your doctor's name.

Next, your doctor is required to fill out the physician certification form. This asks for your doctor's license number, address, phone number, and license type. Your doctor will also choose which of the qualifying conditions he believes is a proper fit for you. This form asks for your name, address, the qualifying condition that the doctor believes you they think you have, and a check box of whether or not you would like to use medical cannabis as a primary form of treatment.


Register

You can register for a Missouri medical marijuana card by filling out the application form and the physician certification form. The application form is available on the Department of Health and Senior Services website, while the physician certification form is available on this site as well.


Application

Once you have filled out the application form and the physician certification form, you will need to submit them. You can mail them in or you can take them to your county health department.

In recent decades, public opinion on marijuana has shifted dramatically: 58% of Americans believe that recreational use should be legal according to Gallup polls (compared to 12% who felt this way about 30 years ago).

Though cannabis has not been legalized for recreational use in Missouri, the state passed three laws this past May to allow for legal medical marijuana. Patients can now buy CBD oil without fear of prosecution after showing a doctor's note.

Missouri Qualifying Conditions

In order to obtain a medical marijuana card in Missouri, you must first undergo an evaluation by a certified physician. During that evaluation, the physician must confirm that you have at least one qualifying condition.

The list of qualifying conditions is always expanding, but here is where it stands now:

  • Cancer
  • Terminal illness
  • Epilepsy
  • Glaucoma
  • Intractable migraines
  • Chronic medical condition(s) with severe and persistent pain or muscle spasms, including but not limited to multiple sclerosis, Parkinson’s Disease, Tourette’s Syndrome, or seizures
  • Debilitating psychiatric disorder(s), including post-traumatic stress disorder (PTSD)
  • Human Immunodeficiency Virus (HIV) or Acquired Immune Deficiency Syndrome (AIDS)
  • Alzheimer's Disease
  • Cachexia
  • Chronic medical condition(s) usually treated with prescription medication(s) that involve the risk of physical and/or psychological dependence
  • Any other source(s) of chronic pain or any other debilitating medical condition(s) a physician would deem suitable for medical marijuana treatment, including Amyotrophic Lateral Sclerosis (ALS), inflammatory bowel disease, Crohn’s Disease, autism, neuropathies, sickle cell anemia, or wasting syndrome

Age & Residency

Only Missouri residents can apply for a medical marijuana card. You will need to provide your Missouri State I.D. or driver’s license as well as proof of residency for this purpose. You will also need to be at least 18 years old.


Medical Documentation

In order to get a Missouri medical marijuana card, you must provide documentation from a licensed physician stating that you have a qualifying medical condition. A few of the most common conditions include cancer, epilepsy, and multiple sclerosis. However, any condition that is treated with cannabis may be qualified.


Get Evaluated

In order to get a Missouri medical marijuana card, you will need to undergo an evaluation by a licensed physician. This evaluation will determine whether or not you have a qualifying medical condition. The physician will also provide documentation stating that you have a qualifying condition.

For the sake of safety and convenience, it’s advisable to undergo this evaluation at a virtual appointment with your physician.
