Patients who meet the requirements for medical marijuana must have a Missouri-licensed doctor complete their Physician Certification Form, which can be done via a virtual appointment.
Patients can utilize the Standard Physician Certification Form to request the usual monthly dose of four ounces. If a patient needs more than the customary four ounces, they should complete the Alternative Physician Certification Form. They must submit two of these forms, each signed by a different doctor but with the same qualifying medical condition checked.
A Parent/Legal Guardian Consent Form must be completed and submitted with the application of patients who are minors (under the age of 18).
Register
Qualifying patients must register with the Missouri Department of Health and the Department of Health and Senior Services (DHSS) before they may apply for a medical marijuana card. To do so, follow these simple steps:
- 1) Go to medicalmarijuana.mo.gov to access the Missouri medical marijuana website.
- 2) Select the "Patient Information" link.
- 3) Select "Patient - How to Apply."
- 4) Select "Registration of Patient/Caregiver Application"
- 5) In the pop-up window requesting authorization to reroute to the patient registry page, select "Yes."
- 6) Click "Register" in the top right corner of the sign-in screen.
- 7) Enter your name, email address, phone number, social security number, and birthdate on the registration form.
Application
Through its website, Missouri's DHSS offers a simple application procedure for medical marijuana users.
Patients who meet the requirements must submit identifying information in their online applications (name, date of birth, social security number, and mailing and email addresses). Then, a number of documents must be submitted, including evidence of Missouri residency, a doctor's certification form that is filled out and signed, a copy of a government-issued photo ID, and a crisp, color headshot.
It should be noted that doctors will electronically transmit the certification form, and patients must attach the form to their application.
The application must be signed and dated, and all fees must be paid as the last step.
