To begin applying for a Pennsylvania card, you will need to register online on the PA DOH website. The registration form will ask for your contact information as well as proof of Pennsylvania residency. You can submit a driver's license or ID card as proof of residency, but make sure this document has your most current address. The system will give you a patient ID number; save it and bring this number to your appointment.
Next, you will need to receive certification for a medical marijuana card from a doctor who has been certified with the PA Department of Health. You will need to log into the same account you used to register and submit a $50 fee. Once the PA DOH receives your registration fee, it will mail your card to the address listed on your driver's license or ID card.
