In order to get a medical marijuana card in Ohio, you need to fill out the forms and provide the necessary documentation. The forms are available on the website of the Ohio Board of Pharmacy.
The required documentation includes a copy of your driver's license or state ID, a copy of your proof of residency, and a copy of your most recent tax return. You also need to provide a statement from your doctor confirming that you have a qualifying medical condition.
If you are a resident of Ohio and would like to apply for a medical marijuana card, you will need to visit the state's website and register for an account. You will then need to provide some basic information about yourself, including your name, date of birth, and Social Security number. You will also need to provide information about your primary care physician.
