The third required form is called "Request for Application", and you can find it on the Ohio Department of Health website. Once your application is approved, you will need to provide this form to the dispensary where you purchase your medical marijuana.
Several other forms may be required during the application process (e.g., financial information for patients applying for a reduced fee card).
Register
