There are a few forms that you will need in order to get a medical marijuana card in Ohio. You will need to provide your proof of residency, proof of identity, and proof of age. You can find the full list of required forms on the Ohio Medical Marijuana Control Program website.
In order to get a medical marijuana card in Cuyahoga Falls, Ohio, you will need to provide some information to the state. This includes your name, date of birth, and social security number. You will also need to provide proof of residency in the form of a utility bill or bank statement. Once you have provided this information, you will need to pay a fee.
