There are a few different forms that you will need to fill out in order to get a medical marijuana card in Ohio. The first form is the Patient Registration Form, which can be found on the Ohio Medical Marijuana Control Program website. You will need to provide your name, date of birth, and social security number on this form.
The second form is the Physician Certification Form, which must be completed by a certified physician. This form confirms that you have a qualifying medical condition and that medical marijuana would be beneficial for treating that condition.
The third form is the Declaration of Caregiver Form, which must be completed if you are appointing a caregiver to help you access and use medical marijuana. This form requires your name, the name of your caregiver, and a brief statement confirming that the caregiver will be able to help you access medical marijuana if needed.
Register
In order to get a medical marijuana card in Ohio, you will need to register with the state. You can do this online or by mail. To register online, you will need to create an account on the Ohio Medical Marijuana Control Program website. You will then be able to fill out the registration form and submit it electronically.
If you choose to register by mail, you can download the registration form from the website or call (614) 387-0547 to have one mailed to you. You can then mail the completed form to the address listed on the form.
Application
When it comes to getting an Ohio medical marijuana card, the process can be a little confusing. This is especially true if you've never applied for a card before. Here are the steps you need to take to register for an Ohio medical marijuana card:
- Get a copy of your driver's license or state ID.
- Gather your medical records.
- Fill out the application form.
- Pay the application fee.
