Step 1: Identify Your Qualifying Condition

  • Proof of New York Residence: acceptable forms include a utility bill dated within 60 days of your application, a signed and current lease, mortgage agreement or property deed, or a pay stub dated within 60 days of your application
  • Photo: A portrait-oriented .jpg or .jpeg image between 18KB and 4GB that displays your head and shoulders, selfies are acceptable, photos with filters or of your driver's license are unacceptable
  • Proof Of Name Change: If you've legally changed your name, you must submit a copy of the official court judgment, marriage certificate, or updated driver's license or non-driver ID card
  • Proof of Identity: You must submit a copy of your valid driver's license or non-driver ID card

Step 4: Submit Your Application

Once you have the certificate and compile your documents, you can begin your application by establishing an MY.NY.GOV account. After logging in, access the "health applications" tab on your dashboard and enter the Medical Cannabis Data Management System.

You can then electronically link your certificate using your DMV ID number, complete the guided application, and upload your supplemental documents. Be sure to complete all fields of the application, check your responses for accuracy, and ensure all your uploaded documents are clear and legible. The OCM has a complete list of acceptable documents.

Step 5: Receive Your Medical Marijuana Card

Once approved, you will be able to immediately access a temporary medical marijuana card through your online account. You will receive your official registry card by mail within 7 business days of approval.

In New York State, you and/or your designated caregiver can begin purchasing medical marijuana products as soon as you’re approved. These products can include vapes, tinctures, edibles, topicals, and a variety of other items containing THC and/or CBD. You must present your card, government-issued ID, and marijuana certificate at each dispensary visit. Also, be sure to carry your registry identification card and certificate whenever you possess marijuana in public.
